The problem with a voice activated translation app is that it needs some training to the speakers voice to have a chance of getting the correct translation. So just hearing a voice once or twice and with different english accents all over the world it will not be able to translate correctly or give out some really weird stuff.

Not that I have found. But there are plenty of translator aps around that will bring it up in writing. Italian is simple to read and pronounce and will sound a lot more pleasant if you do it yourself rather than play someone a computerized voice.

Sadly offline translation isn't there yet, but offline line maps you have a few options, try Mapswithmylite and MapDroyd. Those are both for Android. The best offline translation app for Japanese text only is still Jibbigo. Hope that helps.
